Insurance and Fees
We are happy to assist you with all necessary documentation for services provided which are typically covered under your insurance. We are not directly contracted with your insurance company, however we will provide the paperwork needed to submit for out-of-network reimbursement. Payment in full is required at the time of service.
Deductibles
Patients with high deductibles are essentially fee-for-service patients until their deductibles are met. We are happy to offer transparent pricing so that you may fully understand charges prior to your visit. Our appointments are comprehensive and no additional charges will be added to the charges listed below. Itemized receipts will be provided to you so you may mail them to your insurance company for application toward your deductible.
iNSURANCE & FEES
We are happy to provide you with an insurance coded, itemized receipt required by your insurance company for out-of-network reimbursement. Although we are not contracted with insurance companies, we will happily provide you with a receipt so that you may submit your claims to your insurance company.
Medicare Patients
We are not a contracted Medicare provider, however we are happy to see all patients under our pricing plan below. For past Medicare patients, please call for pricing update.
Health Savings Accounts (HSA)
Our services are qualified medical expenses for HSA accounts. We provide a receipt that you can keep for your records.

*Comprehensive exam pricing includes up to four medically necessary biopsies and liquid nitrogen treatment for precancerous lesions. Most patients typically require one comprehensive exam yearly, high risk patients are evaluated biannually and those with recent melanomas are discussed at the time of diagnosis.
